- Copy of the Child Care Immunization Certificate (Form 3231)
- Copy of the Birth Certificate
- Copy of Parent’s or Legal Guardian’s Driver License
- Copy of Ear, Eye, and Dental Screening (4 years old and up)
- Copy of Any Court Orders Regarding Child Custody/ Visitation Issues
